Background on Google Forms:
An activity like this is extremely easy to set up.
The survey is actually a Google Form. If you have a Google Account (if you have a GMail account or a YouTube account then you have a Google account) then you have Google Drive.

Go to your Drive and choose Form to create your survey. When you create a form you will automatically generate a results sheet.

If you get stuck, go to YouTube, search for 'using Google Forms' and watch a video on how to do it!
